A qualitative study exploring perspectives towards rational use of medicines in Pakistan's Malaria Control Program (MCP) BJPS
Malik,Madeeha; Hassali,Mohamed Azmi; Shafie,Asrul Akmal; Hussain,Azhar.
Malaria is one of the most important global public health problems threatening the health of the population owing to prevailing socio-economic conditions and epidemiological reasons in Pakistan. This qualitative study has focused on the perspectives held towards the rational use of medicine intervention among malaria control program officials. Eight semi-structured interviews with all officials working for the malaria control program in Islamabad were conducted. The interviews, which were audio-taped and transcribed verbatim, were evaluated by thematic content analysis and by all authors. All respondents agreed on successful implementation of the malaria control program in Pakistan for controlling malaria by improving diagnostic and treatment facilities...

ZIMMERMAN,Robert H; PAULINO,Francisco Xavier; ARRUDA,Mercia de.
From January 19 to February 25, 1997 an entomological survey of the seringais and larger towns along the Acre and Purus rivers was made as part of the project “Revisitando a Amazônia de Carlos Chagas; da Borracha a Biodiversidad”. Eleven anopheline species and 1285 specimens were collected landing on human baits. The four most abundant species were Anopheles albitarsis s.l. (n=778), A. darlingi (n=359), A. rangeli (n=69) and A. oswaldoi (n=60). A total of 252 larvae were collected of which 10 anopheline species were identified. The most abundant species collected were A. albitarsis s.l. (n=88), A. deaneorum (n=45) and A. triannulatus (n=40). The low numbers of Anopheles collected and the absence of the principal malaria vector A. darlingi at the seringais...

ACT Now or Later: The Economics of Malaria Resistance AgEcon
Laxminarayan, Ramanan.
In the past, malaria control efforts in sub-Saharan Africa have relied on a combination of vector control and effective treatment using chloroquine. With increasing resistance to chloroquine, attention has now turned to alternative treatment strategies to replace this failing drug. Although there are strong theoretical arguments in favor of switching to more expensive artemisinin-based combination treatments (ACTs), the validity of these arguments in the face of financial constraints has not been previously analyzed. In this paper, we use a Bioeconomic model of malaria transmission and evolution of drug resistance to examine questions of optimal treatment strategy and coverage when drug resistance places an additional constraint on choices available to the...

Acute respiratory distress syndrome due to vivax malaria: case report and literature review BJID
Lomar,André V.; Vidal,José E.; Lomar,Frederico P.; Barbas,Carmen Valente; Matos,Gustavo Janot de; Boulos,Marcos.
Severe pulmonary involvement in malaria has been frequently reported in cases of Plasmodium falciparum infection, but rarely in vivax malaria. Among the 11 previous cases of vivax-related severe respiratory involvement described in the literature, all except one developed it after the beginning of anti-malarial treatment; these appear to correspond to an exacerbation of the inflammatory response. We report the case of a 43-year-old Brazilian woman living in a malaria-endemic area, who presented acute respiratory distress syndrome (ARDS) caused by P. vivax before starting anti-malarial treatment. The diagnosis was made based on microscopic methods. A negative rapid immunochromatographic assay, based on the detection of Histidine Rich Protein-2 (HRP-2) of P....

Agricultural efficiency, malaria incidence and health expenses among Ugandan farmers AgEcon
Ulimwengu, John M.; Badiane, Ousmane.
The importance of health in promoting economic development has been forcefully stated by the World Health Organization’s Commission on Macroeconomics and Health. In this paper, we look at the impact of own household health expenses on malaria incidence and ultimately on agricultural efficiency. We use a non-parametric method to estimate agricultural efficiency, therefore avoiding the issue of identification of the proper household agricultural production function. In addition the simar-wilson approach followed in this paper accounts for bias induced by serial correlation among farmers. A Tobit model with endogenous health production function is used to estimate the impact of malaria incidence on agricultural efficiency. Data come from the 2006 National...

Malaria and schistosomiasis are major infectious causes of morbidity and mortality in the tropical and sub-tropical areas. Due to the widespread drug resistance of the parasites, the availability of new efficient and affordable drugs for these endemic pathologies is now a critical public health issue. In this study, we report the design, the synthesis and the preliminary biological evaluation of a series of alkoxyamine derivatives as potential drugs against Plasmodium and Schistosoma parasites. The compounds (RS/SR)-2F, (RR/SS)-2F, and 8F, having IC50 values in nanomolar range against drug-resistant P. falciparum strains, but also five other alkoxyamines, inducing the death of all adult worms of S. mansoni in only 1 h, can be considered as interesting...

An in vivo test to assess mefloquine 25 mg/kg for the treatment of uncomplicated falciparum malaria in Rondônia, Brazil BJID
Basano,S.A.; Bianco,A.; Taylor,W.R..J; Olliaro,P.; Camargo,L.M.A..
Drug-resistant Plasmodium falciparum is undermining malaria control efforts worldwide. In Brazil, mefloquine (MQ) at a dose of 15 mg/kg body weight is used to treat P. falciparum. At this dose, MQ resistance developed rapidly in Thailand. Use of a higher MQ dose may retard the development of resistance. We treated 50 patients aged one to 67 years who had acute, uncomplicated P falciparum malaria using MQ 25 mg/kg. There were no serious adverse events. Two patients complained of dizziness and insomnia. Assessing evaluable patients, the day 42 cure rate was 40/42 [95.2% (95% confidence interval 83.8 to 99.4%)]. Mefloquine was efficacious and well tolerated in this small cohort from the state of Rôndonia.

Aspectos atuais da distribuição geográfica de Anopheles (Kerteszia) cruzii Dyar & Knab, 1908 no estado do Espírito Santo, Brasil Entomología
Rezende,Helder Ricas; Cerutti Junior,Crispim; Santos,Claudiney Biral dos.
Anopheles cruzii was registered in Espírito Santo state both in 1957 and 1968. Our study provides additional evidence of its presence, including municipalities that were non mentioned before. Anopheles captures undertaken between 1998 and 2003 pointed out a distribution restricted to the forested outskirts of the mountains in the highland area of the state. Its epidemiological role is suggested by the simultaneous presence of malaria cases in the same municipalities.

Association between Knops blood group polymorphisms and susceptibility to malaria in an endemic area of the Brazilian Amazon Genet. Mol. Biol.
Fontes,Aparecida Maria; Kashima,Simone; Bonfim-Silva,Ricardo; Azevedo,Rochele; Abraham,Kuruvilla Joseph; Albuquerque,Sérgio Roberto Lopes; Bordin,José Orlando; Langhi Júnior,Dante Mário; Covas,Dimas Tadeu.
Complement receptor 1 (CR1) gene polymorphisms that are associated with Knops blood group antigens may influence the binding of Plasmodium parasites to erythrocytes, thereby affecting susceptibility to malaria. The aim of this study was to evaluate the genotype and allele and haplotype frequencies of single-nucleotide polymorphisms (SNPs) of Knops blood group antigens and examine their association with susceptibility to malaria in an endemic area of Brazil. One hundred and twenty-six individuals from the Brazilian Amazon were studied. The CR1-genomic fragment was amplified by PCR and six SNPs and haplotypes were identified after DNA sequence analysis. Allele and haplotype frequencies revealed that the Kn b allele and H8 haplotype were possibly associated...

Candidacy of liver stage antigen-1 for Plasmodium falciparum vaccine development OAK
Taylor-Robinson, Andrew W.; Heal, Karen G..
Malaria is the world's most important parasitic disease, imposing a massive health burden on people living in the tropics, often in the poorest countries. The vast majority of deaths in humans from malaria are caused by one species of the hemoprotozoan, Plasmodium falciparum., against which effective control measures are urgently needed. The global situation has deteriorated in recent times due to increased resistance of the anopheline mosquitoes that transmit P. falciparum to insecticides, and of the parasites themselves to drug therapy. An efficacious and cost-effective vaccine against this parasite is considered a public health priority. A vaccine that targets pre-erythrocytic parasites in the liver could potentially prevent clinical disease by blocking...

Characterization of Plasmodium falciparum glutamate dehydrogenase-soluble antigen BJMBR
Rodríguez-Acosta,A.; Domínguez,N.G.; Aguilar,I.; Girón,M.E..
The major aim of this study was to characterize a soluble Plasmodium falciparum antigen from the plasma of malaria-infected humans and Plasmodium falciparum culture supernatants, using immunoabsorbent techniques and Western blotting. An Mr 60-kDa protein was isolated from the plasma of patients with Plasmodium falciparum malaria by affinity chromatography using rabbit anti-Proteus spp GDH(NADP+) serum as ligand. This protein, present in plasma of patients with acute Plasmodium falciparum infection, in Plasmodium falciparum culture supernatants, and in immune complexes, was tested with Plasmodium falciparum malaria hyperimmune serum from patients living in hyperendemic areas and rabbit anti-Proteus spp GDH(NADP+) serum prepared in the laboratory. In this...

Clinical aspects of hemolysis in patients with P. vivax malaria treated with primaquine, in the Brazilian Amazon BJID
Ramos Júnior,Wilson M; Sardinha,José FJ; Costa,Mônica RF; Santana,Marli S; Alecrim,Maria GC; Lacerda,Marcus VG.
This report describes the development of hemolysis in eighteen glucose-6-phosphate dehydrogenase deficient patients treated for Plasmodium vivax malaria with chloroquine and primaquine. The most frequent findings accompanying hemolysis were fever and leukocytosis, in addition to anemia requiring red blood cell transfusion, and development of acute renal failure. Hemolysis in patients using primaquine is not infrequent and contributes to the morbidity of infection caused by Plasmodium vivax.

Comparison of the diagnosis of malaria by microscopy, immunochromatography and PCR in endemic areas of Venezuela BJMBR
Rodulfo,H; De Donato,M; Mora,R; González,L; Contreras,C.E.
Whole blood samples (N = 295) were obtained from different locations in Amazonas and Sucre States, in Venezuela. Malaria was diagnosed by microscopy, OptiMAL™ and polymerase chain reaction (PCR), with Plasmodium vivax, P. falciparum, and P. malariae being detected when possible. We identified 93 infections, 66 of which were caused by P. vivax, 26 by P. falciparum, and 1 was a mixed infection. No infection caused by P. malariae was detected. The sensitivity and specificity of each diagnostic method were high: 95.7 and 97.9% for microscopy, 87.0 and 97.9% for OptiMAL, and 98.0 and 100% for PCR, respectively. Most samples (72.2%) showed more than 5000 parasites/µL blood. The sensitivity of the diagnosis by microscopy and OptiMAL decreased with lower...

Current Antimalarial Therapies and Advances in the Development of Semi-Synthetic Artemisinin Derivatives Anais da ABC (AABC)
PINHEIRO,LUIZ C.S.; FEITOSA,LÍVIA M.; SILVEIRA,FLÁVIA F. DA; BOECHAT,NUBIA.
ABSTRACT According to the World Health Organization, malaria remains one of the biggest public health problems in the world. The development of resistance is a current concern, mainly because the number of safe drugs for this disease is limited. Artemisinin-based combination therapy is recommended by the World Health Organization to prevent or delay the onset of resistance. Thus, the need to obtain new drugs makes artemisinin the most widely used scaffold to obtain synthetic compounds. This review describes the drugs based on artemisinin and its derivatives, including hybrid derivatives and dimers, trimers and tetramers that contain an endoperoxide bridge. This class of compounds is of extreme importance for the discovery of new drugs to treat malaria.

Does Reducing Malaria Improve Household Living Standards? AgEcon
Laxminarayan, Ramanan.
Living in malaria-endemic regions places an economic burden on households even if they do not actually suffer an episode of malaria. Households living with endemic malaria are less likely to have access to economic opportunities and may have to modify agricultural practices and other household behavior to adapt to their disease environment. Data from Vietnam demonstrate that reductions in malaria incidence through government-financed malaria control programs can contribute to higher household income for all households living in endemic areas. Empirically, a 10% decrease in malaria cases at the national level translates to a roughly US $30 million annual economic benefit in the form of improved living standards.

Epitopic Specificity of the Human Immune Response to the Invariant Region of a Polymorphic Plasmodium faciparum Merozoite Surface Antigen OAK
Rzepczyk, C. M.; Anderson, K. L.; Crurshes, P. A.; Baxter, E. P.; Kere, N.; Irving, D.; Dyer, S.; Jones, G. L..
Merozoite surface antigen 2 (MSA2) is a Plasmodium falciparum vaccine candidate. Antibody responses to MSA2 in Melanesians naturally exposed to P. falciparum were investigated by ELISA using the recombinant MSA2 protein known as Ag 1609 and 65 overlapping synthetic peptides spanning predominantly the conserved N and C terminal regions of MSA2. Significant differences were obtained between control and malaria exposed subjects in the ability of sera to bind to Ag 1609. The median absorbance obtained with control sera was 0.36 whereas that obtained with sera from malaria exposed subjects was 1.8. The sera of ninety-five percent of malaria exposed subjects reacted significantly with Ag 1609. The percentage reactivity of the control sera with Ag 1609 was 39%....
